French outlet LaVdn have suggested an interview with the Chelsea star will be released on Wednesday evening.
Speaking in an interview to be broadcast by RMC Sport, the Belgium international was asked about his future and said: “I know what I am going to do.
“I have made a decision.”
On Twitter, Belgian football expert Kristof Terreur confirmed Chelsea are aware of what Hazard wants to do.
He wrote on Twitter: “Eden Hazard already knows for a while what he wants. Club know what he wants too.”
Hazard has made no secret of his desire to play for Real Madrid but also insisted he could stay at Chelsea.
But the 28-year-old is in the final 18 months of his contract with Chelsea and is no closer to putting pen to paper on a new deal.
Real are seeking a replacement for Cristiano Ronaldo, who left the Bernabeu last summer.
And Thibaut Courtois, who made the move from Chelsea to Real last year, has urged Hazard to do similar.
“I [would] choose Eden, of course,” Courtois told Belgian newspaper Het Nieuwsblad.
“I really like him, but the choice is up to the board.
“I do not know what will happen. That is not a question for me, but for the chairman and the management.”
Hazard has had one of his best season’s for Chelsea with 13 Premier League goals and ten assists.
Last week, Chelsea boss Maurizio Sarri said if Hazard wanted to leave, he wouldn’t stand in his way.
“Eden is 28. If he wants to go, I think he has to go,” Sarri said.
“Of course, I hope the opposite, I hope he wants to stay here.
“He has the potential to be the best player in Europe at the moment.”
